Registration Dates are currently being set for the 2012-2013 school year and will be posted soon. Forms that are active below are ready to be used for the 2012-2013 school year unless otherwise indicated. BFHS no longer mails registration forms. Parents and guardians can find these forms linked below, as well as in the BFHS Main Office. For your convenience, many of the forms are interactive so you can complete them before printing. All forms needed must be completed and sign prior to coming to Registration. Unfortunately, we will have to ask you to step out of line to take care of this if the forms are not completed.

You can pay most fees on-line through e~Funds on the links below, or you can choose to pay at registration via cash or check.

The athletics forms for sports physicals and alternate year cards are now combined into one form. Parents and students should complete and sign the upper portion of the form as usual, then choose which box applies for the sports physical. Rule of thumb: Incoming freshmen, juniors and transfer students need a sports physical. If you have filed a sports physical with the Athletic Dept. after 4/1/2011, you don't need a physical, and you would check the alternate year box (usually sophomores and seniors.)
